Dear WPPD Alum:
Thank you for coming to the WPPD Alumni Survey Site. Below is an informed consent that is required by the University of Florida when such surveys are done. Please review the informed consent statement below and select the “Continue” button if you agree to participate.
Please come back and visit the WPPD alumni site again! Once the survey is completed, we will post a summary of the results.

Protocol Title:
WPPD Program Evaluation: Survey of Graduates
Please read this consent document carefully before you decide to participate in this study.
This study involves research related to the Working Professional Pharm.D. Program at the University of Florida.
Purpose of the research study:
The purpose of this study is to evaluate how the Working Professional Pharm.D. (WPPD) program, has impacted your career since graduation and how the College of Pharmacy can meet graduates’ future learning needs. Such evaluations are now required for program accreditation and the results will be used for accreditation purposes.
What you will be asked to do in the study:
Each individual who has graduated from the WPPD program will be asked to complete an anonymous online survey.
Time required:
The survey is expected to take approximately 15 minutes.
Risks and Benefits:
The information identified on this survey will not be reviewed by anyone other than the research team. Your participation will not be tracked and your responses will remain anonymous. The survey results will be used to identify the strengths of the WPPD program and areas for improvement.
There are no anticipated risks since all graduate information will remain anonymous.
Compensation:
Participants will not be compensated for participation.
Confidentiality:
Your identity will be kept confidential to the extent provided by law. All responses will be anonymous. Your name will not be used in any reports.
Voluntary participation:
Your participation in this study is completely voluntary.
Right to withdraw from the study:
You do not have to answer any questions in the survey that you do not wish to complete. You also have the right to withdraw from the survey at anytime without consequence.
