Proof of Enrollment and/or Degree
Enrollment and degree verifications are occasionally requested by parents, insurance companies, scholarship providers, employers and loan companies who require proof of your student standing, that is, whether you are currently enrolled or not.
Current College of Pharmacy Students
Requests can be made to the registrar's office in person, by fax or by mail. All verifications from Registrar's Office contain: your name, class and college, major, dates of attendance, enrollment status by term, and honors and awards received from UF. A cumulative grade point average can be provided if requested. Any release of academic information to a third party requires the student's permission, whether by signature or by electronic certification.

Present a picture ID, such as your Gator 1 Card. The request will be processed while you wait.

Verifications will be mailed within five working days to the address specified on the form. There is no fee.
OFFICE FOR STUDENT AFFAIRS
If the third party will not accept verification from the University of Florida, and must receive documentation only from the College of Pharmacy, please fill out the following form. Verifications can be mail, faxed, or picked up after two working business days from the College of Pharmacy Student Affairs Front Office (HPNP G232).

The National Student Clearinghouse also provides enrollment and degree verifications for a minimal charge.
Former College of Pharmacy Students and Alumni
Former students and alumni can also request proof of enrollment and degree to the registrar's office, in person, by fax or by mail.
