Instructor evaluations

Welcome to the Course Instructor Evaluation instruction page. This page consists of six sections. Please read them in order. There is a lot to read. It will take five to ten minutes. However, when you are done reading the pages, completing the instructor evaluations will be a breeze. Each of the links (i.e., highlighted in blue AND underlined) within this document will be active. If you double click on the link, it will take you to the appropriate page within the College of Pharmacy's Online Course Evaluation system. To get back to the page that you are now reading, just use the "back" function (or its equivalent) of your browser.
The course and instructor evaluation system is an important aspect of your course participation. Each week, you will receive an email from the College that provides you with a progress report. So, it is very important that the College has your correct email address. The progress report will inform you of whether you have "completed" a current evaluation, whether one is "pending", or whether you are "late". This progress report is meant as a reminder and, hopefully, will serve to prompt you to complete the evaluations on a timely basis. It is our hope that by completing these evaluations as the semester progresses, rather than all at the end of the semester, you will not feel as rushed and will be able to provide thoughtful information for the instructor. Okay, here's how you do it.
1. Go to the College of Pharmacy home page. You see two "frames". The frame on the right is larger than the frame on the left. Scroll down the left-hand frame until you reach the link entitled "Teacher Evaluations". Double click on the link.
2. You should now be at the "Teacher Evaluations Link" page. In the box that appears (see below), you should see several choices, including Search, COP Faculty Eval(-uation) Form and COP Course Eval(-uation) Form. Double click on the COP Faculty Eval Form link.
3. In the box that appears (seen below), you should see the Faculty Evaluation Form. The first box requests your Social Security Number (SSN). Course evaluations are confidential. Neither your identity nor your individual responses to the statements will be revealed to anyone on the faculty. Since the instructor evaluations are required, we need a method of tracking whether you have completed the evaluations or not. That is the purpose of asking for your SSN. Place your SSN into the box, DO NOT INCLUDE the hyphens. If you include the hyphens, you may not receive credit for completing the evaluation.

The next box is the Course and Section selections. At the start, you should see Choose One in the box. Place your cursor over the downward pointing triangle and click. Each course and section offered in the college is represented in the drop down menu. All of the courses and the course section numbers that you are individually responsible for evaluating are provided in the weekly email from the college. Scroll down to the appropriate course and section number and release the mouse. The name of the course and sections should appear and remain in the box.
IMPORTANT. Many courses in the college have multiple sections. Many times each section has a separate instructor. Other times the same instructor is responsible for multiple sections. It is very important that you select the correct course and section number. If you do not select the correct section number, you may not get credit for completing the instructor evaluation. If you do not know your section number, please refer to the weekly email. It will contain the course number and section number for which you are registered. Select the course number and section number from the weekly email.
The next box is the Instructor box. At the start, you should see Choose One in the box. Place your cursor over the downward pointing triangle and click. The course instructors' names are provided in the weekly email from the college. Each instructor in the course you have selected is represented in the drop down menu. Scroll down to the appropriate instructor and release the mouse. The instructor's name you have selected should remain in the box. If an instructor is not represented in the drop down box, please type their name in the Other box. Multiple instructors may be listed for a single course throughout the semester. That means that multiple instructors teach in the course. Each instructor should have a date after their name. Evaluate that instructor after the date listed in the email. The instructor's teaching commitment should be completed for the class after that date.
4. Directly under the Other box you should see the response categories for the evaluation statements or the Key. These statements are the most important section of the course evaluation form. The response categories ask you how well you feel that the instructor did each of the activities, ranging from 1=poorly to 5=excellent. The statements are divided into sub-sections. Two subsections ask closed-ended questions about (a) course instructor questions that are consistent across the campus for comparison purposes and (b) College instructor questions that are specific to the College of Pharmacy. Part III, the optional questions section, asks for open-ended responses.
For the closed-ended, instructor statements, place your cursory over the radio button response category that most closely reflects your opinion. Click the radio button and a "dot" should appear in the circle. If you want to change your mind, just move your cursor over the new radio button and click. The dot should move from the previous button to the new one. If you do not have an opinion about the statement or choose not to share it, please leave the statement blank and do not click any of the radio buttons for the statement.
The Optional Questions section asks for your open-ended comments about (a) what were the strengths of this instructor's teaching? (b) what were the weaknesses of this instructor's teaching? and (c) what teaching improvements would you suggest to the instructor? PLEASE, PLEASE, PLEASE. Leaving the comment boxes blank does not give the course instructor information he or she might use to improve their teaching. It is important to identify aspects of the instructor's teaching that you like, as well as those aspects you do not like. At the same time, comments like "I didn't like anything about the instructor" does not give the instructor much information to base improvements upon either. If you did not feel the instructor was approachable, what was "unapproachable" about him or her? Were they never in their office during office hours? Or, did you choose not to approach them because you felt they were unapproachable and what did you base your feeling upon? If they did not teach course material in a way that was relevant to the practice of pharmacy, give evidence for your opinions and examples of how it could be improved. A thoughtful and helpful comment is invaluable in the faculty's efforts to continually improve the curriculum. Harsh, hurtful, and rude comments, and personal attacks are not helpful. In fact, they are counterproductive.
5. IMPORTANT! Please be certain that you type in your SSN correctly. If you do not type your SSN into the box correctly, you may not receive the appropriate credit for completing the course evaluation. The weekly email message from the College gives you a listing of the instructors for whom you have completed an evaluation and for whom you have NOT yet completed an evaluation form. The weekly course and instructor evaluation reminders will include the "due" date. For example, Instructor Jones is lecturing in Course PHAXXX for three weeks during the semester. Your weekly email message will include the date that Instructor Jones' teaching commitment in the class is complete. At the end of Intructor Jones' lectures, please complete the teaching evaluation. Instructors' teaching commitments in a class will end at different times throughout the semester. So, you will have to keep ever vigilant. It also allows you to complete the evaluations at your choice of time and allows you to have sufficient time for reflection to give the instructor valuable feedback.
6. Since evaluations are a course requirement for each course, the College of Pharmacy wants to facilitate completion of this important requirement as much as possible. An important part of the process is the weekly email that you will receive during the semester notifying you of the courses and instructors. IMPORTANT! Please be certain that the College has your current email address. This email reminder will be sent to the email address that was last registered with the Office for Student Affairs. If you have not received any email messages throughout the semester from the college, please consider the possibility that the Office for Student Affairs may not have your current email address. If you do not complete a course evaluation for a course, you will receive an "I" or an incomplete grade for the semester because you have not completed all of the course requirements.
