Case Study 5

A patient receives an antimicrobial dose of 300 mg as i.v. bolus every 8 hr. After steady state is reached, a peak level of 30 µg/ml is determined; the level 4 hr after the peak is 9 µg/ml.

(1) Estimate ke for the antimicrobial in this patient.

(2) Calculate the patient’s Vd.

(3) What will be the average concentration at steady state?

(4) What will be the trough plasma level?

(5) If we want to increase the average concentration at steady state without changing the dose, what would we do? How will the peak plasma level be affected accordingly? Explain.

(6) What dose is required to attain a peak plasma level of 40 mg/ml?

(7) If a 500 mg dose is given every 8 hr, what will be the resulting Cpeak and Ctrough?
