Advanced Pharmacy Practice Experience Evaluation System

In the 2002-2003 Rotation Cycle we started using an online evaluation system.

The system is web based and is tied directly to our scheduling database so that it is refreshed every night at 12:00 midnight. A side benefit of this is that this link will also provide you with an up-to-date version of your schedule.

To enter the system you will need your UFID. The entry screen looks like this:

Once you enter your UFID and hit the "submit query" button you will be taken to a web page which will look something like this:

Each evaluation which has not be completed will be shown in RED. After you complete an evaluation it will turn to GREEN so you will always know which ones you have completed. To complete an evaluation just click on the "Evaluate" button. This will take you to an evaluation form specific to the rotation you are evaluating.

The list shown is effectively your schedule. You will notice however that your 8 week rotations Adult Medicine and Ambulatory Care are only show once. The reason for this is that even though they are 8 weeks long they are still just one course, have a single course number and receive a single grade. Therefore there is only 1 evaluation done for the entire 8 weeks of these rotations.

Please be pay close attention to the KEY for the questions. if the question is not applicable leave it blank.

Since the accuracy of the evaluation page is based on the accuracy of our database it is important for us to be notified if the data shown does not match with what you thought you were supposed to have. If this occurs please notify us and we will determine the problem.
